He will offer them a free consultation at Kissdental Alderley Edge for assessment (subject to £30 deposit).

• Patient Details (Name, DOB, Address, Email, Tel number)

• A summary of any previous treatment on the tooth

• Any radiographs available of the tooth

• Please state if the patient is in pain and needs urgent treatment
